Abstract U cilju ispitivanja različitih aspekata kvalitete života bolesnika s limfoproliferativnim zloćudnim bolestima, standardiziranim upitnicima prikupljeni su podaci od 51 bolesnika. Od toga 6 bolesnika (11,7%) oboljelih od Hodgkinovog limfoma (HL), dvadeset i osam (54,9%) od NHL visokog stupnja malignosti (HG-NHL), osmero (15,7%) od NHL niskog stupnja malignosti (LG-NHL) te njih devet (17,7%) od kronične limfocitne leukemije (CLL). Rezultati su statistički analizirani hi-kvadrat testom za male uzorke te prikazani grafički.
Rezultati pokazuju da najmanju prosječnu životnu dob imaju oboljeli od HL (prosjeku 34,8 godina) a najstariji su oboljeli od kronične limfocitne leukemije ( prosječno 71 godinu). Također je nešto više muškaraca u svim skupinama osim HG NHL dok su prema ostalim sociodemografskim podacima sve 4 skupine izjednačene i usporedive.
U skupini HL gotovo svi ispitanici u nekoj mjeri osjećaju iznenadni umor ( 67%), no psihološki aspekti bolesti su značajno izraženiji kao npr.promjene raspoloženja (33%), zabrinutost za zdravlje u budućnosti (50%) i zabrinutost za recidiv ( 50%). Oboljeli od KLL su u najmanjoj mjeri zabrinuti za svoje zdravlje dok su oboljeli od HG NHL najviše od svih skupina zabrinuti za svoje zdravlje u budućnosti (71% ) boje se da bi se bolest mogla vratiti (53%), brinu se da bi mogli postati ovisni o drugima (57%), zabrinuti su za dugoročne posljedice bolesti i liječenja (preko 50 % brine da bi terapija mogla uzrokovati buduće zdravstvene probleme).
Ovo istraživanje pokazuje da je percepcija kvalitete života oboljelih od limfoma, premda postoji jasna distribucija pojednih važnijih ili manje važnih domena u skupinama, ustvari individualna i predstavlja osobnu subjektivnu procjenu. Stoga je sve jasnija potreba za ovakvom procjenom, i to od strane samih pacijenata, kako bismo mogli poboljšati kvalitetu njihovog života, a što će u budućnosti predstavljati jedan od važnih ciljeva našeg liječenja.
Abstract (english) The aim of this study was to analyse various quality of life domains in lymphoma patients by collecting standardized questionnaires specific for the type of limphoproliferative disease. In toto 51 pts: 6 (11,7%) with Hodgkin disease (HL), 28 (54,9%) with high grade non Hodgkin lymhoma (HG-NHL), 8 (15,7%) with low grade NHL and 9 (17,7%) with chronic lymphocytic leukemia (CLL). Results have been analized by hi2 test for small samples and showed graphically.
Results confirmed that the youngest group was HL (34,8 y), the oldest group, pts with CLL (71 y) with higher prevalence of men in all groups except HG NHL, while acording all other charachteristics all groups were comparable.
In HL group almost all of pts feel some degree of sudden fatigue ( 67%), but psychological aspects were much more pronounced in HL pts i.e. mood changes (33%), worry for the health in the future (50%) and worry about possible relaps ( 50%). Patients with CLL were worried the less among all groups about their health issues, while pts with HG NHL, the most among all groups have been worried about their health in the future (71% ), they were afraid of relaps (53%), and have concerns that thay could become dependent on others (57%). Also they expressed their concerns for long term repercussions and side effects of the illness and its treatment (more than 50 % thinks that therapy could cause health problems in the future).
This survey, altough included relatively small group of the patients, implicated that perception of the quality of life in lymphoma patients is very individual and subjective, but there is clear distribution of important domains and differences among the groups. So, there is obvious need for such an evaluation by standardized qouestionaires in lymphoma patients (patient reported outcome) with final purpose to enhance their QoL. In the future it will become one of the most important goals in medical treatment and therapy in lymphoma patients.
