Abstract Esencijalni tremor (ET) i Parkinsonova bolest (PB) spadaju u progresivne neurodegenerativne bolesti koje se odlikuju motornim i ne–motornim simptomima. Tremor im je predominantan znak koji ujedno spada u najčešće poremećaje pokreta uopće te dovodi do veće ili manje nesposobnosti u obavljanju svakodnevnih životnih aktivnosti, što pak vodi do smanjenja kvalitete života; uključujući fizičke i psihosocijalne aspekte zdravlja.
Cilj ovog diplomskog rada je bio procijeniti kvalitetu života vezanog za tremor kod pacijenata oboljelih od esencijalnog tremora i Parkinsonove bolesti.
Ispitanici i metode: Ovo prospektivno randominizirano istraživanje je obuhvatilo 40 ispitanika koji su podijeljeni u dvije skupine po 20 ispitanika prema dijagnozi bolesti u razdoblju od 1. travnja do 1. kolovoza 2018. godine. Ispitanici su ambulantnih odrasli pacijenti Klinike za neurologiju, Kliničkog bolničkog centra Rijeka sa sigurnom dijagnozom esencijalnog tremora i Parkinsonove bolesti te su dobrovoljno pristali na sudjelovanje u provođenju istraživanja, ispunjavanjem upitnik za tremor QUEST. Upitnik u cijelosti sadrži 46 pitanja s unaprijed formuliranim odgovorima i uputama za popunjavanje te se sastoji se od dva dijela. Osim standardiziranog upitnika obradila se i medicinska dokumentacija. Za potrebe obrade podataka korišten je računalni program Statistica.
Rezultati: Ispitanici obje skupine postižu nizak ukupan quest indeks (QSI) te on iznosi 17,5 za oboljele od esencijalnog tremora i 13,2 za oboljele od Parkinsonove bolesti. Ispitanici iz obje skupine imaju najveći rezultat na podljestvici Hobiji što govori u prilog tome da tremor u najvećoj mjeri utječe na kvalitetu hobija i slobodno vrijeme. Oboljeli od Parkinsonove bolesti najmanje su nezadovoljni segmentom psihosocijalna kvaliteta života. Kod oboljelih od esencijalnog tremora kvaliteta života podjednaka je po segmentima te iznosi od 14,7 (Psihosocijalna kvaliteta) do 19,6 (Hobiji). Muškarci i žene u ovom uzorku ne razlikuju se značajno u kvaliteti života, zdravstvenom statusu, procjeni tremora i kvaliteti prema QUEST upitniku.
Zaključak: Tremor u manjoj mjeri utječe na kvalitetu života iako ispitanici obje skupine postižu nizak ukupan quest indeks, što znači da su ispitanici općenito vrlo zadovoljni postignutom kvalitetom života.
Abstract (english) Essential tremor (ET) and Parkinson's disease (PB) are part of progressive neurodegenerative diseases characterized by motoric and non-motoric symptoms. Tremor is a predominant sign that is at the same time the most frequent movement disorder and leads to greater or less disability in carrying out daily life activities, which leads to a reduction in the quality of life; including physical and psychosocial aspects of health.
Objective: The aim of this research paper was to evaluate the quality of life associated with tremor, researched on patients with essential tremor and Parkinson's disease.
Patients and methods: This prospectively randomized study comprised 40 participants divided into two groups of 20 respondents according to disease diagnosis from 1. April to 1. August 2018. Subjects were ambulant adult patients of the Clinic for Neurology, Clinical Hospital Center Rijeka with a diagnosis of essential tremor and Parkinson's disease. They agreed to participate in conducting the research by filling in QUEST questionnaire. The questionnaire contains 46 questions with pre-formulated responses and filling instructions, consisting of two parts. In addition to the standardized questionnaire, medical records were processed. Data processing was used by the computer program Statistica.
Results: Both groups achieved a low total or quest summary index (QSI) 17,5 for patients with essential tremor and 13,2 for Parkinson's disease. Respondents from both groups have the highest score on Hobby's hypothesis, which suggests that tremor largely affects the quality of hobbies and leisure time. Parkinson's patients less dissatisfied with the segment of psychosocial quality of life. The quality of life for patients with essential tremors, equals per segments from 14,7 (Psychosocial Quality) to 19,6 (Hobbies). Men and women in this sample do not differ significantly in quality of life, health status, tremors and quality estimates according to the QUEST questionnaire.
Conclusion: Tremor rarely affects the quality of life, although respondents of both groups achieve a low overall quest index, meaning that respondents are generally very satisfied with the quality of life achieved.
