Abstract
Kontrastna sredstva na bazi joda i gadolinija se koriste svakodnevno u većini radioloških pretraga. Imaju veliko značenje pri postavljanju točnih dijagnoza i u većini slučajeva se smatraju sigurnim i učinkovitim. Međutim, njihova upotreba nije bez rizika jer se pojavljuju reakcije na kontrastna sredstva koje mogu biti opasne po život. Nuspojave variraju od manjih fizioloških i blagih alergijskih reakcija do rijetkih, ali teških i po život opasnih reakcija. Važno je identificirati pacijente s čimbenicima rizika kako bi se vjerojatnost za neželjenom reakcijom smanjila. Iako je rasprostranjenost ovih reakcija mala, reakcije na kontrastna sredstva se javljaju, stoga je potrebno znati kako se manifestiraju i kako ih brzo liječiti. Istraživanje je provedeno pretraživanjem sustava ISSA te IBIS-a od datuma 1.1.2015. do 1.1.2020. na Kliničkom zavodu za radiologiju Kliničkog bolničkog centra Rijeka. Na temelju toga pronađeno je 34 pacijenta. Istraživanje se temeljilo na odnosu muškaraca i žena koji razvijaju reakciju preosjetljivosti, dobnim skupinama, podjeli stupnja ozbiljnosti reakcije, simptomima reakcija, terapiji koju primaju bolesnici te vrsti i volumenu kontrastnoga sredstva. Odnos muškaraca i žena od ukupno 34 pacijenta, rezultirao je s podjednakim brojem muškaraca i žena. Najviše reakcija razvili su ispitanici u dobnoj skupini između 61. i 70. godine starosti. Najučestalije reakcije su bile blagog stupnja odnosno kod 76,47% ispitanika, dok je zabilježena samo jedna reakcija teškog stupnja. Osvrnuvši se na terapiju koju su primali ispitanici nakon reakcije uočeno je da su se koristile različite vrste lijekova. Sukladno s time došlo se do zaključka da ne postoji uniformna terapija za liječenje reakcija preosjetljivosti na kontrastno sredstvo. Analizirajući podatke uočili smo da je najveći broj ispitanika razvio reakciju pri CT pregledima to jest 73,53%.
Abstract (english)
Iodine and gadolinium-based contrast agents are used daily in most radiological examinations. They are of great importance in making accurate diagnoses and in most cases are considered safe and effective. However, their use is not without risks because there are reactions to contrast agents that can be life-threatening. Side effects range from minor physiological and mild allergic reactions to rare but severe and life-threatening reactions. It is important to identify patients with risk factors in order to reduce the likelihood of an adverse reaction. Although the prevalence of these reactions is small, reactions to contrast agents occur, so it is necessary to know how they manifest and how to treat them quickly. The research was conducted by searching the ISSA system and IBIS from 01/01/2015 to 01/01/2020 at the Clinical Department of Radiology of the Clinical Hospital Center Rijeka. Based on this, 34 patients were found. The study was based on the relationship between men and women who developed a hypersensitivity reaction, age groups, the division of the severity of the reaction, the symptoms of reactions, the therapy received by patients and the type and volume of contrast medium. The male-female ratio of a total of 34 patients, resulted in an equal number of men and women. Most reactions were developed by subjects in the age group between 61 and 70 years of age. The most frequent reactions were mild, ie in 76.47% of subjects, while only one severe reaction was recorded. Looking back at the therapy received by the subjects after the reaction, it was observed that different types of drugs were used. Accordingly, it was concluded that there is no uniform therapy for the treatment of hypersensitivity reactions to a contrast agent. Analyzing the data, we noticed that the largest number of subjects developed a reaction on CT examinations, ie 73.53%.
