Abstract
Kronični subduralni hematomi su jedno od najčešćih neurokirurških stanja. Iako u većini slučajeva nema podataka o traumi u anamnezi, neki slučajevi mogu nastati kao posljedica koagulacijskih defekata, intrakranijalne hipotenzije i upotrebe antikoagulacijske i antitrombocitne terapije. Do povećanja CSDH dolazi zbog rekurentnog krvarenja, nakupljanja CSF i povećane eksudacije iz vanjske membrane hematoma. ˝Burr hole˝ kirurška drenaža je indicirana kod nekompliciranih CSDH, dok kod asimptomatskih i visokorizičnih pacijenata koristimo nekirurške metode, primjerice steroidnu terapiju. CSDH su povezani sa nekolicinom komplikacija među kojima su najčešća ponovna javljanja hematoma, infekcije, tenzijski pneumocefalus, moždani edem, epileptički napadaj kao i nemogućnost reekspanzije mozga.
Abstract (english)
Chronic subdural hematoma (CSDH) is one of the most common neurosurgical conditions. Although definite history of trauma could be obtained in a majority of cases, some cases may be secondary to coagulation defect, intracranial hypotension, use of anticoagulants and antiplatelet drugs. Recurrent bleeding, increased exudates from outer membrane, and cerebrospinal fluid entrapment have been implicated in the enlargement of CSDH. Burr-hole surgical drainage is the treatment of choice for an uncomplicated CSDH, while nonsurgical treatment like steroid theapy is reserved for asymptomatic and high risk patients. CSDH could be associated with recurrence, infection, tension pneumocephalus, cerebral edema, seizure and failure of the brain to expand.
