Abstract
Leukemija je zloćudna bolest matičnih hematopoetskih stanica. Etiologija bolesti je nepoznata iako su poznati čimbenici koji povećavaju rizik od nastanka leukemije. Leukemije se prema kliničkoj slici dijele na akutne i kronične. Prema staničnom tipu dijele se na mijeloidne i limfoidne. Klinička obilježja leukemije proizlaze iz infiltracije koštane srži koja dovodi do anemije, neutropenije i trombocitopenije. Kao rezultat javljaju se nespecifični simptomi poput slabosti umora, otežanog disanja, glavobolje, gubitka težine i krvarenja. Akutna limfoidna leukemija obično nastaje u djece dok se akutna mijeloidna leukemija češće javlja kod odraslih. Kronična mijeloidna leukemija se povezuje sa izlaganjem zračenju i kemikalijama i uglavnom se javlja kod starijih pacijenata. Kronična limfoidna leukemija se otkriva slučajno, a asimptomatska faza bolesti može trajati godinama. Oralne manifestacije leukemije javljaju se češće kod akutnih leukemija. Intraoralni pregled pokazuje znakove bljedila sluznice, spontana krvarenja, petehije, ekhimoze, hiperplaziju desni, infekcija (virusnih, bakterijskih i gljivičnih), mukozitisa i oralnih ulceracija. Terapija oralnih manifestacija temelji se na pravilnoj oralnoj higijeni. Terapijske opcije liječenja leukemije su: kemoterapija, transplantacija hematopoetskih matičnih stanica ili palijativna i potporna terapija. Doktor dentalne medicine ima ulogu u poboljšanju kvalitete života pacijenta i dijagnostici ranih znakova bolesti.
Abstract (english)
Leukemia is a malignant disease of the hematopoietic stem cells. Etiology of the disease is unknown although there are known factors that increase the risk of leukemia. Leukemia is classified into acute and chronic. According to the cell type, they are divided into myeloid and lymphoid. Clinical features of leukemia are caused by the infiltration of the bone marrow causing anemia, neutropenia and thrombocytopenia. As a result, nonspecific symptoms such as weakness, fatigue, difficulty breathing, headache, weight loss and bleeding occur. Acute lymphoblastic leukemia is commonly occurring in children while acute myeloid leukemia is more common in adults. Chronic myeloid leukemia is associated with exposure to radiation and chemicals and mostly occurs in older patients. Chronic lymphocytic leukemia is detected accidentally, and the asymptomatic stage of the disease may last for years. Oral manifestations of leukemia occur more frequently in acute leukemia. Intraoral examination shows signs of pallor, spontaneous bleeding, petechiae, ecchymosis, gingival hyperplasia, infections (viral, bacterial and fungal), mucositis and oral ulcerations. Oral manifestation therapy is based on proper oral hygiene. Therapeutic options for treating leukemia include: chemotherapy, hematopoietic stem cell transplantation or palliative and supportive therapy. Dentists play a role in improving the patient's quality of life and diagnosing early signs of the disease.
