Abstract
Patološka veza između usne šupljine i maksilarnoga sinusa, koja se pojavljuje nakon raznih patoloških promjena ili terapijskih postupaka, naziva se oroantralna komunikacija. Najčešće dolazi do komunikacije prilikom ekstrakcije maksilarnih molara zbog anatomije maksilarnoga sinusa u tim područjima te zbog divergentnih i dugih korjena samih molara. U slučaju nastajanja komunikacije potrebno je poduzeti odgovarajuće mjere kako bi se spriječile moguće daljnje komplikacije. Dvije najčešće komplikacije su postoperativni maksilarni odontogeni sinusitis i kronična oroantralna fistula. Najčešće se oroalntralna komunikacija dijagnosticira pomoću Valsalvina testa. Postoje različite tehnike terapije koje se koriste za zatvaranje oroantralne komunikacije. Za komunikacije veće od 7 mm tradicionalno se koriste lokalni mukoperiostalni režnjevi (bukalni i palatinalni režanj). U većini slučajeva dolazi do spontanoga zatvaranja komunikacije, bez potrebe za kirurškom terapijom i dodatnih komplikacija.
Abstract (english)
Oroantral communication is a pathological communication between the maxillary sinus and the oral cavity that occurs during various pathological abnormalities or treatments. In most cases, the communication is caused during the extraction of maxillary molars due to the anatomic form of the maxillary sinus in that area and divergent roots of the maxillary molars. If there is communication, appropriate measures must be taken to prevent possible further complications. Two most common complications are the postoperative maxillary sinusitis and the chronic oroantral fistula. The diagnosis of oroantral communications is made by Valsalva test. There are many different therapy tecniques used for the closure of the oroantral communication. Traditionally for closure of oroantral communications larger than 7 mm, local mucoperiostal flaps (buccal and palatal flap) are used. Commonly the communication heals spontaneously without any need for surgical procedures and no additional complications.
