Abstract
Cilj: Cilj ovog istraživanja bio je utvrditi učestalost metaboličkog sindroma među bolesnici s akutnim pankreatitisom te njegov utjecaj na klinički tijek bolesti, pojavu lokalnih i sistemnih komplikacija te ishod bolesti. Ispitanici i postupci: Provedeno je retrospektivno istraživanje u koje su uključeni svi bolesnici sa dijagnozom akutnog pankreatitisa postavljenom temeljem revidiranih Atlanta kriterija, hospitalizirani u Kliničkom Bolničkom centru Rijeka od 1. siječnja 2016. do 31. prosinca 2017. godine. Rezultati: Istraživanje je uključilo 298 ispitanika od kojih 41 ima metabolički sindrom. Statistički značajne razlike među skupinama bolesnika koji imaju metabolički sindrom i onih koji nemaju su nađene u vrijednostima koncentracije ureje i kreatinina u serumu, pojavi nekroze gušterače (12/41 naspram 38/257; P = 0,037) i vrijednosti APACHE II bodovnog sustava . Nije utvrđena značajna razlika u učestalosti smrtnog ishoda, organskog zatajenja i lokalnih komplikacija između bolesnika s i bez metaboličkog sindroma. Također, težina pankreatitisa nije se značajno razlikovala u ovisnosti o broju prisutnih pojedinih komponenti metaboličkog sindroma. Zaključak: Ovim istraživanjem nismo potvrdili utjecaj metaboličkog sindroma na razvoj težih oblika akutnog pankreatitisa. Preciznija istraživanja zahtjevaju uključivanje većeg uzorka ispitanika.
Abstract (english)
Aim: The aim of this study was to determine the frequency of metabolic syndrome among patients with acute pancreatitis and its effect on clinical course of the disease, the occurrence of local and systemic complications and the outcome of the disease. Patients and methods: A retrospective study was conducted involving all patients with acute pancreatitis diagnosis based on revised Atlanta criteria, hospitalized at the Clinical Hospital Center of Rijeka from 1 January 2016 to 31 December 2017. Results: The survey included 298 subjects, 41 of them with metabolic syndrome. Statistically significant differences between groups of patients with metabolic syndrome and those without were found in urea and creatinine serum concentrations, pancreatic necrosis (12/41 vs 38/257, P = 0.037) and APACHE II score system. There was no significant difference in the incidence of fatal outcome, organic failure and local complications between patients with and without metabolic syndrome. Also, the severity of pancreatitis did not differ significantly depending on the number of individual components of the metabolic syndrome. Conclusion: This study did not confirm the influence of metabolic syndrome on the development of more severe acute pancreatitis. More precise research requires inclusion of a larger sample of respondents.
