Abstract
Većina farmakoloških sredstava za sada nije učinkovita u sprečavanju post-ERCP pankreatitisa. Jedina sredstva koja imaju dokazanu djelotvornost su nesteroidni protu-upalni lijekovi (NSAIL) koje također treba vrlo pažljivo i selektivno primjenjivati. Za dokazivanje mogućih pozitivnih učinaka ostalih sredstava trebalo bi napraviti još mnogo detaljnih istraživanja. Svaka ERCP tehnika sa sobom nosi određeni rizik. Naravno, vrlo je bitno procijeniti kada taj rizik premašuje korist i obratno. Tehnike koje su se pokazale najbolje i najmanje štetne su postavljanje stentova i žicom vođena kanulacija. Međutim vrlo je bitno procijeniti koji je pacijent kandidat za određenu tehniku, a koji nije. Pacijente s povećanim rizikom od nastanka post-ERCP pankreatitisa svakako treba detaljno obraditi i na njima primijeniti odgovarajuću tehniku. Za sada ne postoji niti univerzalno farmakološko sredstvo niti univerzalna tehnika ERCP-a za koju bi se sa sigurnošću moglo reći da je primjenjiva na sve ili bar veliku većinu pacijenata. Pristup i dalje mora ostati individualan i najveća je odgovornost na liječniku da procijeni koja je metoda prevencije najbolja za njegovog pacijenta.
Abstract (english)
The most pharmacological agents now are not effective in the prevention of post-ERCP pancreatitis. The only funds that have proven efficinecy are non sterodial anti-inflammatory drugs (NSAIDs) which also need to be carefully and selectively applied. To demonstrate the potential positive effects of other agenst more detail investigations should be made. Each ERCP tehnique carries some risk. Of course it is very important to estimate when the risk excide the benefit and vice versa. The tehniques that has been proven best and least harmful are certainly placing stents and wire guided cannulation. However, it is important to asses wich patient is a candidate for particular tehnique and wich is not. Patients with increased risk of post-ERCP pancreatits certainly should be detaily procesed and apply them appropriate tehnique. For now there aren't a universal pharmacological agent or a universal technique ERCP which would certainly be said that is applicable to all or at least the vast majority of patients. Access must still remain the largest individual and the responsibility of the physician to assess which is the best method of prevention for his patient.
