Abstract
Zbog nezadovoljstva vlastitim osmijehom pacijenti sve češće posjećuju doktore dentalne medicine. Kod analize pacijentovog osmijeha, u ovisnosti o vidljivosti gingive, linija osmijeha može biti niska, srednja (1-3 mm vidljive gingive) ili visoka (više 4-5 mm). Neestetski izgled vidljive gingive je različite etiologije (nepravilni tijek gingivnog ruba, prekomjerna vidljivost gingive, hipertrofija gingive, pojačana potrošnja zubne strukture i stvaranja efekta kratkih kruna zubi). Sve te nepravilnosti su indikacije za terapijski postupak produljenja kliničke krune zuba. Za produljenje kliničke krune zuba bitna su dva faktora: širina keratinizirane gingive (ne smije biti manja od 2 mm) i visina dentogingivalnog kompleksa (prosjek 4-5 mm aproximalno, 3 mm bukalno od ruba alveolarne kosti). Ovisno o njima moguće su tri kliničke situacije. Jednostavniji zahvati gingivektomije (kirurško uklanjanje tkiva gingive) i gingivoplastike (remodeliranje gingive) rade se u slučaju dovoljne visine dentogingivalnog kompleksa i nema potrebe za dodatnom obradom kosti. U slučaju nedovoljne visine dentogingivalnog kompleksa (manje od 3 mm koronalno od ruba alveolarne kost) izvode se i ostektomija i osteoplastika. U slučaju nedovoljne širine keratinizirane gingive (manje od 2 mm, nema viška gingive) potrebna je apikalna repozicija režnja. Kod takvog zahvata nema dovoljno pričvrsne gingive za gingivektomiju. Produljenjem kliničke krune zuba mogu se rješiti mnogi problemi vezani uz estetiku osmijeha bez obzira na njihovu etiologiju.
Abstract (english)
Dissatisfaction with their own smile is the main reason for patients to visit their dentist. In the analysis of the patient's smile, depending on the visibility of the gingiva, smile line can be low, medium (1-3 mm of visible gingiva) or high (over 4-5 mm). Unaesthetic appearance of visible gingiva is caused by different etiology (irregular flow of the gingival margin, excessive visibility of gingiva, gingival hypertrophy, increased consumption of tooth structure that creates the effect of short teeth crown). Two factors are important for this procedure. For the extension of the clinical crown two factors are important: the width of keratinized gingiva (shall not be less than 2 mm) and height of dentogingival complex (an average of 4-5 mm aproximaly, 3 mm buccaly from the edge of the alveolar bone). Depending on factors three clinical situations are possible.Simpler procedures like gingivectomy (surgical removal of gingival) and gingivoplasty (remodeling of the gingiva) are made in the case of sufficient height of dentogingival complex and there is no need for additional treatment of the bone.In case of insufficient height of dentogingival complex (less than 3 mm coronally from the edge of the alveolar bone) osteotomy and osteoplasty are performed. In case of insufficient width of keratinized gingiva (less than 2 mm, there is no excess of gingiva) apical flap reposition is required. In such procedure there is no sufficient attached gingiva for gingivectomy.
