Abstract
Vrtoglavica je jedan od najčešćih neuroloških simptoma koja pogađa veliki broj svih dobi. Bolesnici s vrtoglavicom se najčešće javljaju liječniku obiteljske medicine koji onda moraju razlučiti ima li pacijent uopće vrtoglavicu ili se radi o simptomu neke druge organkse bolesti i ako je riječ o vrtoglavici je li ona perifernog ili centralnog podrijetla. Često je to izazov za liječnike zbog subjektivnosti simptoma i poteškoća u definiranju vrtoglavice. Pacijenti često predsinkopalno stanje, omaglicu i nestabilnosti doživljavaju kao vrtoglavicu, što može krivo usmjeriti liječnika u postavljanju dijagnoze. Liječnik obiteljske medicine, koji je postavio radnu dijagnozu vrtoglavice, najčešće pošalje pacijenta neurologu koji onda razlučuje o kojoj je vrtoglavici riječ-perifernog ili centralnog podrijetla. Kako su uzroci većine vrtoglavica relativno bezopasni, a trajanje simptoma samoograničavajuće, postavljene dijagnoze mogu ne biti revidirane. U ovom radu pregledno su prikazane karakteristike centralnih i perifrernih vrtoglavica, potom dostupni epidemiološki podaci, a potom rezultati retrospektivnog epidemiološkog istraživanja podataka Hrvatskog zavoda za javno zdravstvo i bolničke baze podataka Ambulante za vrtoglavice, Klinike za neurologiju, KBC Zagreb. Vrtoglavice perifernog podrijetla su češće nego one centralnog podrijetla. Većina autora navodi benigni paroksizmalni pozicijski vertigo (BPPV) kao najčešći uzrok vrtoglavice. Vrtoglavice najčešće opterećuju stariju populaciju, iako od nje mogu bolovati i mlađi ljudi. Procijenjuje se da polovica bolesnika starijih od 75 godina pati od vrtoglavice.
Abstract (english)
Vertigo is one of the most common neurological conditions that affects a lot of older people. Patients with vertigo often visit primary care physician who has to resolve if the patient has vertigo or is his or hers symptom a sing of organic disease. If the patient really has vertigo, the physician has to decide if vertigo is peripheral or central. That is often a challenge for physicians because the symptoms are very subjective and vertigo is difficult to identify. Patients often mistake presyncopy, light-headedness and imbalance with vertigo, which can misdirect the physician in the making of the diagnosis. The primary care physician who made the first diagnosis usually sends the patient to see a neurologis. Because most causes of vertigo are relatively harmless and the symptoms are self-limiting, the diagnosis can be not revised. In this paper, the characteristics of peripheral and central vertigo are reviewed, followed by available epidemiologic data and results of retrospective epidemiologic study of data from Croatian Institute of Public Health and hospital database of Infirmary for dizziness, Department of Neurology, University Hospital Centre Zagreb. Peripheral vertigo is more common than central. Most authors agree that benign paroxysmal positional vertigo (BPPV) is the most common cause of vertigo. Vertigo is most common in older patients, although it can occur at any age. It is estimated that half of the patients older than 75 suffer from vertigo.
