Abstract
Ovaj diplomski rad obrađuje temu osteoartritisa kuka. Osteoartritis (OA) najčešća je zglobna bolest i najčešći uzrok kronične fizičke nesposobnosti osoba starije životne dobi. Budući da incidencija raste s dobi, a sve je veći udio starijih osoba za očekivati je da će doći do porasta prevalencije osteoartritisa. Uzrokom osteoartritisa smatra se oštećenje uzrokovano mehaničkim stresom zajedno sa smanjenom regeneracijom zglobova. Uzastopni stres na površini hrskavice povezan je sa staničnim i molekularnim promjenama koje su uključene u patogenetski mehanizam nastanka OA. Najčešći simptomi OA kuka su bol i smanjeni opseg pokreta. OA kuka najčešće postupno napreduje, a simptomi se često zanemaruju sve dok nisu zahvaćene aktivnosti svakodnevnog života. Osteoartritis kuka je klinička dijagnoza koju je često moguće dijagnosticirati samo prema kliničkoj slici, ali radiološki nalaz može biti koristan za potvrdu dijagnoze ili za praćenje bolesti. Liječenje bolesnika s osteoartritisom kuka možemo podijeliti na konzervativno, farmakološko i kirurško. U bolesnika s blagim OA bez izraženih simptoma, zadovoljavajući učinak može se postići redukcijom tjelesne mase, vježbanjem i paracetamolom. U uznapredovanom stadiju bolesti nužno je farmakološko liječenje jednim ili kombinacijom analgetika. Ukoliko konzervativnim liječenjem ne možemo postići odgovarajuću kontrolu simptoma, pristupa se kirurškom liječenju.
Abstract (english)
This graduate thesis deals with the topic of hip osteoarthritis. Osteoarthritis (OA) is the most common joint disease and a leading cause of disability among the elderly. Since the incidence of OA is increasing with age and the proportion of older people is constantly increasing, it is expected that the prevalence of OA will rise. The mechanical stress accompanied with decreased cartilage regeneration has been recognized as a cause of OA. Repetitive stress at the articular surface has been associated with cellular and molecular changes involved in the pathogenesis of OA. The main symptoms are pain and limited range of motion. Typically, osteoarthritis develops gradually. The symptoms are often ignored until they affect daily activities. Hip osteoarthritis is a clinical diagnosis which can be diagnosed just by physical examination but radiographic imaging is useful for confirmation or follow up. Treatment options can be divided into conservative, pharmacological and chirurgical. In patients with mild, asymptomatic OA, weight loss, exercise and paracetamol are effective. In cases of advanced OA pharmacological treatment with one or more analgetics is necessary. If conservative management is ineffective, joint replacement surgery may be recommended.
