Abstract
Zdravstveni lokus kontrole (HLC) odnosi se na vjerovanje pojedinca tko ili što ima kontrolu nad njegovim zdravljem. Za osobu koja vjeruje da može utjecati na događaje u vlastitom životu kažemo da ima unutarnji lokus kontrole. Osobe s vanjskim lokusom kontrole vjeruju da su njihove odluke i život posljedica okolišnih čimbenika na koji oni nemaju utjecaj. HLC je jedan od čimbenika koji određuje kako će se pojedinac zdravstveno ponašati. Cilj: Istražiti povezanost između lokusa kontrole i regulacije šećerne bolesti tip 2. Metode: Pretražena je PubMed baza podataka na temelju ključnih riječi i zadanog kriterija o radovima objavljenima između 1990-2016 te su dobivena 74 rada. Pregledom sažetaka tih radova odabrano je 8 radova koji su odgovarali temi. Rezultati: Rezultati upućuju na povezanost unutarnjeg HLC i bolje kratkoročne i/ili dugoročne kontrole šećerne bolesti te bolju samopomoć i samokontrolu ŠBT2 u većem dijelu prikazanih istraživanja. Iako je vanjski HLC bio većinom povezan s lošijim kratkoročnim i/ili dugoročnim pokazateljima kontrole ŠBT2, vjera u zdravstvene djelatnike te vjera u važne druge osobe (obitelj i prijatelji), bila je povezana s pozitivnim učinkom na samokontrolu. Nasuprot tim rezultatima, rezultati dijela istraživanja nisu potvrdili povezanost HLC i regulacije ŠBT2. Zaključak: U konzultaciji s bolesnikom od ŠBT2 liječnici trebaju uzeti u obzir i njegov HLC. Naime, razumijevanjem u što bolesnik vjeruje i kako se ponaša omogućilo bi liječniku da prilagodi mogućnosti liječenja bolesnikovim individualnim potrebama i mogućnostima te tako poboljša ishod liječenja. Poznavanje HLC važno je i zbog razvoja učinkovitijih edukativnih i intervencijskih mjera u liječenju šećerne bolesti.
Abstract (english)
Health locus of control (HLC) refers to the person beliefs who or what has control over his or her health. A person's locus of control is conceptualized as internal when people believe they can control their life or external when they believe their decisions and life are controlled by environmental factors which they cannot influence. Aim: The purpose of this review was to investigate correlation between health locus of control and regulation of type 2 diabetes mellitus (DMT2). Methods: The literature search included Pubmed databases for the years 1990 to the present. Eight articles met all inclusion criteria (DMT2, HLC and disease control). Findings: The Internal HLC was mostly associated with better short-term and/or long-term control of diabetes and better self-control and self-treatment. Although external HLC was mostly associated with poor short-term and/or long-term indicators of diabetes control, faith in health professionals and faith in other important people (including family and friends) was associated with a positive effect on self-control. In contrast to these results, the results of some studies, did not confirm the connection between HLC variables and diabetes regulation. Conclusion: In consultation with DMT2 patients, physicians should take into consideration the HLC variables. Recognizing, what a patient does believe and how he/she behave, would enable them to modify and adjust the treatment modalities to the individual patient abilities and to improve patient outcomes. Understanding LOC of DMT2 patients is important for developing more efficient diabetes menagement.
