Abstract
Rosacea je kronična upalna bolest kože koja zahvaća obraze, nos, bradu i čelo. Najčešće se javlja između 30. i 50. godine života kod osoba svjetlije puti. Glavna klinička obilježa na temelju kojih se donosi dijagnoza su prolazno ili trajno crvenilo lica, teleangiektazije, papule i pustule, fimatozne promjene i okularne manifestacije. To je bolest multifaktorijalne i nerazjašnjene patofiziologije koja je vjerojatno splet genetskih čimbenika, promjena u urođenom imunosnom odgovoru, neurovaskularne disregulacije i utjecaja vanjskih čimbenika, zbog čega klinička prezentacija varira od pacijenta do pacijenta. Iz tog razloga, odabir terapije ovisi o kliničkoj prezentaciji rosacee kod pojedinog pacijenta. Najveću razinu dokaza o učinkovitosti terapije imaju topikalno primijenjeni brimonidin, metronidazol, ivermektin i azelatna kiselina te sistemski primijenjeni doksiciklin i izotretinoin, ali se često propisuju lijekovi čija učinkovitost kod rosacee nije dokazana. Ljekarnici mogu znatno doprinjeti poboljšanju kvalitete života pacijenata savjetovanjem o pravilnoj njezi kože, primjeni lijekova i izbjegavanju okidača.
Abstract (english)
Rosacea is a chronic inflammatory skin disease that affects the cheeks, nose, chin and forhead. It usually starts between 30 and 50 years of age, commonly in fair-skinned individuals. Main clinical characteristics, which are the foundation of a diagnosis, are transient or permanent erythema, telangiectasia, presence of papules and pustules, sebaceous gland hypertrophy and ocular manifestations. The patophysiology of the disease is unclear and multifactorial, probably a combination of genetic factors, native immune response alterations, neurovascular disregulation and environmental factors. Therefore, the selection of therapy depends on clinical presentation of rosacea in each individual patient. Medicines with high-quality evidence of efficacy are locally administred brimonidine, metronidazole, ivermectin and azelaic acid, and orally administred doxycycline and isotretinoin. Still, medicines without proven efficacy in rosacea are often prescribed. Pharmacists can greatly improve patients quality of life by giving the right advice about general skin care measures, administration of the medicines and avoiding triggers.
