Abstract
Gubitak volumena alveolarnog grebena koji se javlja kao posljedica atrofije, parodontnih bolesti ili traume, kao i kongenitalni defekti alveolarnog grebena nastali rascjepom nepca ili uklanjanjem novotvorina, ukazuju na potrebu za koštanom transplantacijom. Materijali za augmentaciju kosti mogu djelovati osteoinduktivno, osteogenetski i osteokonduktivno. Materijali koji se mogu koristiti za augmentaciju su autogeni, alogeni, ksenogeni i sintetski. Autogena kost je i dalje zlatni standard kod augmentativnih zahvata zbog toga što, u usporedbi s aloplastičnim materijalima, posjeduje, osim osteokonduktivnog potencijala, i potencijal osteoindukcije i osteogeneze. Donorska mjesta koja se mogu koristiti za dobivanje autolognog koštanog grafta mogu se podijeliti na intraoralna i ekstraoralna. U intraoralna spadaju ramus mandibule, simfiza mandibule i crista zigomatikoalveolaris, dok se od ekstraoralnih donorskih mjesta mogu koristiti krista ilijaka i kalvarija. Kirurške tehnike autolognim koštanim graftom ovise o vrsti defekta alveolarnog grebena te stoga postoje one s lokaliziranim i opseţnim autolognim koštanim transplantatom, splitted-bone-block tehnika po Fuadu Khouryju i trodimenzionalna „ring“ augmentacija.
Abstract (english)
Bone loss of the alveolar ridge, as a consequence of atrophy, paradental disease or trauma, or congenital defects of the alveolar ridge due to a cleft palate or removal of neoplasm, indicates the need for a bone graft. The materials for bone augmentation may have osteoinductive, osteogenic and osteoconductive properties. The materials that may be used for augmentation are autogenous, allogeneic, xenogeneic and synthetic. Autogenous bone remains the golden standard for augmentative procedures as, in comparison to alloplastic materials, in has osteoconductive, osteoinductive and osteogenic potential. Donor sites used to attain autologous bone graft can be divided into intraoral and extraoral. Intraoral sites include the ramus mandible, symphysis mandibular and crista zygomatico alveolaris, while the most frequently used extraoral donor sites are the cristina iliaca and calvaria. Surgical techniques using autologous bone grafts depend on the type of alveolar ridge defect. These include localised and comprehensive autologous bone transplant, split-bone-block technique according to Fuad Khoury, and tri-dimensional “ring” augmentation.
