Abstract
Dentinska preosjetljivost učestao je klinički problem sa stopom prevalencije u općoj populaciji između 10% i 30 %. Karakterizira je pojava brze i oštre boli eksponiranog dentina kao odgovora na termičke, evaporativne, taktilne, osmotske i kemijske podražaje. Da bi nastala bolna preosjetljivost, dentin mora biti eksponiran, a dentinski tubulusi otvoreni prema oralnom mediju i pulpi. Atricija, abrazija, erozija i gingivalne recesije su najčešći uzroci eksponiranog dentina i preosjetljivosti. Četkanje zubi u kombinaciji s erozivnim kiselinama može dovesti do trošenja cakline zuba i eksponiranja dentina. Za liječenje ovog stanja na tržištu se nalaze brojna sredstva od kojih su neka još u eksperimentalnoj fazi ispitivanja. Dva osnovna principa u liječenju dentinske preosjetljivosti su intereferiranje sa živčanim prijenosom boli i okludiranje otvorenih dentinskih tubulusa. Kao najčešće sredstvo za tretiranje preosjetljivosti koriste se desenzibilizirajuće paste za zube. Najčešći aktivni sastojci desenzibilizirajućih pasti su kalijev nitrat koji djeluje na živčanu provodljivost i fluoridi. U novije vrijeme koriste se i nanočestice hidroksilapatita, kalcij fosfosilikat i aminokiselina arginin u kombinaciji s kalcijevim karbonatom koji okludiraju dentinske tubuluse. Kod težih oblika preosjetljivosti dolaze u obzir restaurativni zahvati i mukogingivalna kirurgija.
Abstract (english)
Dentine hypersensitivity is a common clinical problem with a prevalence rate between 10% to 30%. It is characterized by sharp pain, short in duration, arising from exposed dentine in response to stimuli, typically thermal, evaporative, tactile, osmotic or chemical. In order for the painful hypersensitivity to develop, dentine must become exposed and the dentine tubules must be open to the oral cavity and the pulp. Attrition, abrasion, erosion and gingival recession are the most common causes of dentine exposure and hypersensitivity. A combination of tooth brushing and erosive acid may lead to the wear of enamel and exsposure of dentine. There is a large number of treatment options for managing dentine hypersensitivity, some of which are still experimental. The two main treatment principles are: interruption of the nerve transmission and occlusion of open dentine tubules. The most commonly used desensitizing agents are toothpastes. The majority of desensitizing toothpastes contain as active ingredients potassium nitrate, which interferes with neuro transsmision, and fluoride. Occlusion of dentine tubules has since recently been treated with, nano-hydroxyapatite, calcium phosphosilicate and amino acid arginine with calcium carbonate. In cases of severe sensitivity restorative tretaments and mucogingival surgery can be used.
