Abstract
Kod lomova dugih cjevastih kostiju postoji nekoliko metoda i pristupa kojima moţemo sanirati napuknutu ili slomljenu kost. Metoda izbora na ţalost ponekad ovisi i o plateţnoj moći vlasnika ţivotinje. Konzervativnim metodama lijeĉenja moţemo sanirati samo zatvorene lomove distalno od lakta ili koljena, uporabom razliĉitih imobilizacijskih povoja i udlaga. Prilikom kirurške intervencije moţemo lomove sanirati vanjskom ili unutarnjom fiksacijom. Odabir metode fiksacije ovisi o nekoliko ĉimbenika. Prvi su mehaniĉki faktori procjene loma kosti gdje promatramo stres na alantezu i odnos alanteze i kosti. Kod bioloških faktora procjene loma kosti promatramo procjenjeno vrijeme cijeljenja kosti. Posljednji faktor koji moramo uzeti u obzir je utjecaj vlasnika i pacijenta na cijeljenje loma i njihova volja za suradnjom. Kod vanjskih fiksatora poštujemo naĉela biološke osteosinteze uz minimalan otvoreni ili zatvoreni pristup.Najnoviji tip unutrašnje fiksacije koji se pojavio u posljednjih 15 godina su zakljuĉavajuće ploĉe gdje se zbog oĉuvanja periosta i cirkulacije kosti postiţe brţe cijeljenje rana i smanjuje mogućnost komplikacija nakon operativnih zahvata. Indikacije za uporabu zakljuĉavajućih ploĉa u humanoj i veterinarskoj kirurgiji su kosti loše kvalitete (osteoporotiĉne kosti), kominutivni metafizni ili dijafizni lomovi, periprostetski lomovi (pucanje implantata kod npr. umjetnih kukova), te kompleksni lomovi u zglobovima.
Abstract (english)
In long tubular bone fractures there are several methods and approaches which can rehabilitate bruised or fractured bone. Method of choice unfortunately sometimes depends on owners purchasing power. With conservative threatment we can fix only closed fractures distaly from elbow or knee with immobilisation bandages and splints. During surgical intervention we can fix fractures with external or internal fixation. Choosing the method of fixation depends on several factors. First are mechanical factors where we watch stress on fixed angle implants and fixed angle implant/bone ratio. By biological factors we watch estimated bone healing time. Last factor we have to consider is influence by owner and patient to healing time and their willingness for cooperation. On external fixators we respect biological osteosynthesis principles with minimum open or closed approach Latest internal fixation type which appeared in last 15 years are locking plates where wound healing is faster and possibility of complications are lower after surgery because of periost and bone circulation preservation. Indications for using locking plates in human and veterinary surgery are poor quality bones (osteoporotic bones), comminuted metaphyseal pr dyaphyseal fractures, periprostetic fractures (implant failure in e.g. hip replacement) and complex periarticular fractures.
