Abstract S obzirom da sve više prelazimo na sjedilački način žvota, naša kralježnica se nalazi u neprihvatljivim položajima uslijed čega nastaju razni poremećaji. Sama je kralježnica nosivi stup našeg tijela koji se sastoji od 33 do 34 kralješka koji zajedno čine morfološku jedinicu. Dinamička jedinica kralježnice je vertebralni dinamički segment unutar kojega se događaju svi pokreti, ali i poremećaji. Sami je segment sastavljen od dva dijela, prednjeg i stražnjeg, zbog kojih je pokretljivost moguća. Bitna sastavnica dinamičkog segmenta je intervertebralni diskus, na kojemu se događa najviše promjena. Diskus se sastoji od anulusa fibrosusa i nuklusa pulposusa koji služe kao amortizeri i ravnomjerno raspoređuju silu. Ukoliko sila nje ravnomjeno raspoređena doći će do oštećenja diska te bolnosti u kralježnici. Ovisno o tome koliko se disk ošteti, razlikujemo četiri stupnja diskus hernije. Prvi stupanj ili „bulging“ predstavlja ispupčenje oslabljenog diskusa, ali bez oštećenje anulusa fibrosusa. Kod drugog stupnja (protruzija) dolazi do većeg ispupčenja i djelomičnog defekta anulusa fibrosusa, dok je kod trećeg stupnja ili ekstruzije vidljivo probijanje prstena. Četvrti stupanj jest sekvestracija dijela diska u kanal. Svaki tip hernijacije može ostaviti trenutne posljedice zbog pritiska na korijenove živaca, ali i trajne posljedice.
Stabilitet vertebralnog segmenta kralježnice može biti narušen i određenim traumama koje nastaju zbog prevelikog djelovanja sile na segment. Najčešće su to aksijalna sila ili sila rotacije. Zbog biomehaničkih odnosa među segmentima najčešće se ozljeđuje cervikalna i lumbalna kralježnica, a zbog manje pokretljivost najmanje se ozljeda događa u torakalnom dijelu kralježnice. Ukoliko je došlo do nestabilnog prijeloma kralježnice, trauma se liječi operativno dok se u slučaju stabilnog prijeloma kreće s konzervativnim liječenjem.
Poremećaji dinamičkog vertebralnog segmenta su stanja koja su izlječiva i kao metoda izbora za liječenje su metode fizikalne medicine uz kombinaciju s medikamentoznom terapijom. Medikamentozna terapija se koristi zbog upale i bolnosti koja se događa na mjestu kompresije živaca, dok je glavna ideja liječenje i rehablitacija metodama fizikalne medicine. U to spada elektroterapija, kinezioterapija, termo ili krioterapija, laser te hidroterapija.
Abstract (english) Given that we are increasingly having sedetary way of life, our spine is places in unacceptable positions, what cause various disorders. The spine itself is the supporting pillar of our body which consists of 33 to 34 vertebrae, which together form the morphological unit.
Dynamic spinal unit is the vertebral dynamic segment within which all movements, but also disorders, occur. The segment itself is made up of two parts, front and rear, for which mobility is possible. An important component of the dynamic segment is the intervertebral discus, where most changes occur. Discus consists of anulus fibrosus and pulposus cuffs that serve as shock absorbers and evenly distribute force. If the force is uniformly distributed, it will damage the disc and cause the pain in the spine. Depending on how much the disk is damaged, we distinguish four-stage disc hernias. The first stage or "bulging" is the obstruction of the weak discussion, but without the damage of the fibrosis anule. In the second degree (protrusion) there is a greater degree of protuberance and partial fibrosus anulus defect, while in the third degree or extrusion there is a visible puncture of the ring.
The fourth step is sequencing the part of the disc into the canal.
Any type of herniation can leave the immediate consequences of pressure on the root nerve, but also the permanent damage. The stability of the vertebral segment of the spine may be disrupted by certain traumas that arise due to excessive force acting on the segment. The axial force or rotation force are the most common. Because of the biomechanical relationship between the segments, the cervical and lumbar spine is most often injured, and due to less mobility the least injury occurs in the thoracic spine. If there is an unstable spinal fracture, the trauma is treated operative while in the case of a stable fracture, it moves with conservative treatment.
Dynamic vertebral segment disorders are conditions that can be cured and as a method of choice for treatment we use methods of physical medicine in combination with medication therapy.
Medication therapy is used for inflammation and pain that occurs at the site of nerve compression, while the main idea is treatment and rehablitation by the methods of physical medicine. This includes electotherapy, kinesiotherapy, thermo or cryotherapy, laser and hydrotherapy.
